Insurance Information
As a courtesy, we will submit insurance pre-determination requests on your behalf. This is not a guarantee of any payment and/or reimbursement that you may receive. Unless otherwise specified, the entire fee is due at the time of the procedure. We will inform you of your responsibilities prior to performance of any procedures and help you make an informed decision prior to such time.
Although we will gladly file your claim on your behalf, you may wish to submit the claim yourself. In general, insurers process claims filed directly by patients faster than those filed by the service providers (dental offices).
Further, most dental insurance policies are limited and often only pay for a portion of the procedure(s) that may need to be done.
Co-Payment
The majority of dental plans reimburse approximately 30%-80% of treatment costs. Dr. Linden will work with your insurance company. However, as we do not participate with any insurance plans, the entire fee is due at the time of service. Any necessary forms for your reimbursement will be processed for you as a courtesy.
